Management of Patient Information

It is our top priority to maintain the confidentiality of all patient files and information. We keep patient health information for a period of seven years, and for children, until they reach 25 years of age. At Toronto Family Practice, we ensure that all records are up-to-date and maintained for the appropriate requirement period.

 

If you need a copy of your file, please contact our surgery and speak with our reception staff. Please note that there will be a fee dependent on the size of your patient file.
